She is the founder of Graceful Transitions, where her work is grounded in a single insight: every transition carries loss, and every loss is a transition — and neither has to be navigated alone.
Patricia facilitates Showing Up: Supporting Your Team Through Loss and Transition, along with Living Fully & Dying Prepared, Start the Conversation, and the Legacy Leaders Guide cohort. She hosts the podcast In Conversation with an End-of-Life Specialist and writes Graceful Transitions - an End-of-Life Navigator on Substack. She serves as Board Secretary for the Center for Death Education.
